This questionnaire has been sent to you as a supplier of materials used by Heidelberg Materials UK. As a responsible supplier, Hanson is required to demonstrate that high standards are set and maintained through the raw material supply chain.

If your product is used in construction or construction products it is a legal requirement to provide information under the construction products regulations 305/2011/EU including certificates of constancy of performance, CE Mark Certificate and declaration of performance.

Please complete the sections below where applicable, which cover supplier information, responsible sourcing and product information.

Failure to complete this questionnaire by 30/4/18 will jeopardise your ability to supply Heidelberg Materials UK with your products.
